To stop the dam project, the Mountain Dogs capture the Minister of Construction's son and send a ransom note. The Furude family is told the existence of the parasites and are asked to cooperate with the clinic for research. Research suggests that Rika Furude is the "queen carrier" of the Hinamizawa Syndrome parasites her presence keeps the villagers from developing terminal symptoms and going insane. The Houjou family becomes ostracized by the village due to their support of the dam. The villagers begin protesting while Tokyo starts working to stop the project. Irie Clinic begins construction in Hinamizawa, completed by next spring. The younger sister obtains the demon tattoo and is forced to become Mion while the older sister becomes Shion. Jirou Tomitake is appointed as auditor for the project and Tetsurou Okonogi, leader of the Mountain Dogs is introduced as a supportive military unit. The Irie Institute organization is formed to facilitate research, with Irie the puppet leader while Takano acts behind the scenes. Kyousuke Irie is brought onto the research project due to his knowledge. Self-Defense Force forms the Alphabet Project and shows interest in Hinamizawa Syndrome research, giving funding and support to Takano. Mion Sonozaki and Shion Sonozaki are born as twins. Akane Sonozaki is disowned from the Sonozaki family. The young Takano becomes determined to prove his research legitimate. Hifumi presents a report of his findings into Hinamizawa Syndrome but his research is dismissed as nonsense. Miyoko changes her name to Miyo Takano in honor of him.
